Community Opportunity Center is seeking a dedicated nurse who will be responsible for the review, oversight and assurance that medical needs of COC residents are managed well and care is documented as required by Detroit Wayne Integrated Health Network and Licensing and Regulatory Affairs. The nurse will work collaboratively with the Executive Director, Program Managers and Home Administrators to develop and carry out training for direct support professionals on topics pertaining to medication passing and basic medical care for chronic conditions affecting the COC resident population. The nurse will report to the Executive Director.
Key Responsibilities:
- Program Operations and Oversight
- Visit all COC home sites bimonthly at a minimum to assess the medical status of residents, review medical files and discuss health care with individual residents and their home staff.
- Attend Person Centered Planning meetings for COC residents to discuss health status with the care team and contribute to the plan of care for person served.
- As necessary, for residents with more complicated health challenges, attend resident medical appointments with home staff and the residents to help relay medical information, advocate for the resident and navigate the health care system to seek out the best treatment options.
- Monitor and assist home staff with implementing medical care as outlined by physician and person-centered plan.
- Collaborate with pharmacy and COC staff to monitor resident medications, ensuring residents are receiving the correct medications and the correct therapeutic dose.
- Team Leadership and Development
- Carry out training sessions on medication passing with new COC staff or existing staff who need concepts reinforced.
- Carry out trainings on chronic health conditions experienced by COC residents and impart knowledge to direct support professionals regarding the nature of the illness/condition and proper ways to treat/care for the person with the illness or condition.
- Client and Family Engagement
- Relay information concerning resident health status to parents and families as needed to assist them with care and medical decision making regarding their loved one.

Qualifications:
- Bachelor of Science Degree in Nursing preferred (RN)
- Associate Degree in Nursing Accepted (LPN)
- Some experience caring for people with developmental disabilities.
- Good physical health, including negative TB test.
- Evidence of strong collaborative skills.
- Ability to maintain a flexible schedule.
- Demonstrated ability to relay a positive image to the public.
